Things that can make your breath smell are thing like food (garlic, onions), certain medications, teeth with cavities, smoking, chewing tobacco and dry mouth (need to drink lots of water)

Could be a bad tooth or something you ate. Breath could be coming from your stomach. Also try hydrating since you may have cottonmouth causing it.

I’m sure it is internal problem. Bad breath has many factor, it can be in your sinusitis or in your lungs. The best way is to consult a specialist in that kind.

You seem to have covered all the territory, except for the fact that what you detect as "bad breath" may be coming from your nose, and/or sinuses.
Much depends on your age, and medications you may be taking, or not taking! Have you taken your temperature? Have you had headaches recently, or a cough, or congestion? Are you really fall-asleep tired in the afternoons–unusually so? If you tap each tooth is any one of them tender? Can you ‘taste’ something unpleasant in your mouth at times?
It’s easy to check out your sinuses (not the end all and be all but …) by applying pressure with your fingertips to the following areas: the inner quarter to half of your eyebrows; 1 finger are each side of your nose where glasses would rest, and slide them downward; push 4 fingers into your cheek bones. That isn’t all but the best you can do…and try lying on your stomach and letting your head hang over the edge of the bed … wait a while…if your nose starts to drip, or you have secretions drop into the back of your throat, or your mouth, you may have sinus infection or one coming on…that last trick is great for draining the maxillary (cheek) sinuses because nothing else will …
Again, as others said, you may have a bad tooth – if you can afford a dentist, get to that one and let him/her figure it out for you.
If not, a local dental school, or clinic may be accessed. Where do you go to get your teeth cleaned?
