1. you should probably see your dentist to see if theres anything wrong. You say you dont have gum disease but you never know.
2. If you’re wearing braces at 13, you may be using a Chlorhexidine rinse. That turns your teeth yellow.
3. To solve the yellow problem, again, go see your dentist. Get a checkup, teeth cleaned/scaled. In the meantime, stay away from things that stain, such as COLA, TEA, COFFEE.
4. Bad breath- brush your teeth after meals, try a mouth wash, And a tongue scraper. A tongue scraper will solve a lot of your problems.

Make an appointment with a dentist to see what’s going on with your teeth.
Maybe you should start flossing if you don’t already, food built up in between your teeth could give you bad breath, and could be hard to get out of your teeth. But if gum, mints, mouthwash etc. is not helping, I think it would be best to see a dentist.

Floss everyday before you brush. That should really be all.

You are one of the people who believes that your bad breath is caused by unclean teeth. Wrong! Actually, most bad breath is caused by bacteria on the top of your tongue. Brushing you teeth, using a mint, chewing some gum, etc. will cover up bad breath for a short time but these products do not get rid of bad breath. Look on the top of your tongue, especially in the back for some "white stuff". This stuff must be cleaned off to get rid of bad breath. How you clean your tongue is also important. To get your tongue clean use OraBrush tongue cleaner. I have used one for about 5 months with great success.

Sorry to hear about your problem. It can be frustrating because you’re probably already doing the things you’re "supposed" to do, such as brush and floss. Unfortunately, those things aren’t the true cause of bad breath.
It’s actually a bacteria that "lives" on your tongue. Making that bacteria less plentiful is the only way to improve the condition that I know of. Try the site below for some free advice.

Apple Cider Vinegar can be used as an effective mouthwash that provides long-lasting relief from bad breath. Simply dilute 1/2 tablespoon of the vinegar into a glass of water and gargle it in your mouth for 10 seconds at a time. More useful remedies at http://useinfo5.blogspot.com/
